Primary care doctor's office for older adults on Medicare | Oak Street Health is not your typical doctor’s office. They do things differently in order to offer patients a better primary healthcare experience. Their board certified doctors only see people with Medicare and are therefore focused on the healthcare needs of older adults. They spend time getting to know their patients to develop a comprehensive care plan for their individual needs, and can work with your existing specialist to coordinate your care.
Each clinic features a community room with unique programming including fitness classes, computer lessons, and movie nights. Many of our patients take advantage of our door-to-door transportation options for their appointments.
